New Patient Registration -
If available, please bring your medical card when registering. Alternatively you, or a person authorised to sign on your behalf, will be asked to sign an application form. We will also require photographic ID, for example a student card or driving license and a utility bill for proof of address.
You have a right to express a personal preference regarding the delivery of medical services by our practice staff.
All newly registered patients will be invited to see the practice nurse for a simple health check. If you require the services of an interpreter, please let us know.
Medical treatment is available from the date of registration. Please contact reception for further information.
